Practice Improvement Tool

The Credit Extender-Practice Improvement is designed to increase awareness of the performance improvement measures put forth by the American College of Cardiology (ACC)/American Heart Association (AHA) Task Force on Performance Measures, and the Physician Consortium for Performance Improvement.

The Data Collection Sheet allows participants to track and review their own patient charts and to gather snapshots of their current practice with regard to thromboembolic risk assessment, chronic coagulation therapy, and monthly INR monitoring among patients with atrial fibrillation within their settings.
